It has a diverse population and a specialist Practices – The Doctor Hickey Surgery – a dedicated Practice for homeless people.

Who we are

What we do as a PCN
South Westminster PCN provides a range of innovative services and is running various pilot projects.

Out of hospital services
We provide out of hospital services to help our patients live well at home within their own community. These include Spirometry, Warfarin and Womens’ Health services at specialist hub sites. Other services include ECG, Phlebotomy, Asthma, Mental Health, Diabetes and Children and Young Adults and we also provide Acupuncture and Eating Disorder clinics.

Routine appointments and check-ups
Working alongside experts, we are developing systems that better identify the multiple health needs of patients so we can establish appropriately timed appointments that enable us to manage and monitor health conditions at one extended appointment.

Specialist supporting roles
We work with a number of specialist supporting roles that are available in Primary Care, including Clinical Pharmacists, Health and Wellbeing Coaches and Care Co-ordinators.

Extended Hours
We support all patients across South Westminster PCN by providing extended hours, in addition to our usual hours, at specific locations.

Access to the GP Practice
We are working with experts to improve access to patients’ GP Practices. This includes better understanding access and demand and tailoring our services accordingly.

Patient participation
To ensure patients are kept at the heart of the PCN and our services are in line with local needs, we are looking at ways to develop our Patient Participation Group meetings.

Improving access to your GP
We want to make it easier for patients to get care when they need it.
Earlier this year, our local Integrated Care Board (ICB) – the organisation that plans health services in your local area – requested that we submit a plan for better access. They set goals and gave us money to complete the actions in this plan.
Our plan was approved. It should mean faster care, better use of doctors’ time, and more ongoing care for people who need it most.
