This scheme was introduced in 2019 as part of the Government’s commitment to improving access to Primary Care. At CLH, we believe that healthcare requires a community-centred approach and we recruit ARRS roles that best suit the needs of our local population.

    Advanced Nurse Practitioners

    Advanced Nurse Practitioners, who are educated to master’s level or equivalent, use their advanced skills and knowledge to better meet the needs of the people they care for.

    Care Coordinators work with GPs and other Primary Care professionals to provide personalised care to patients that reflects local Primary Care Network (PCN) priorities and tackles health inequalities.

    First Contact Physiotherapists

    First Contact Physiotherapists are qualified autonomous clinical practitioners who can assess, diagnose, treat and manage musculoskeletal problems and undifferentiated conditions.

    GP Assistants

    GP Assistants arrange clinical support (e.g. referrals, tests and follow-ups), conduct simple clinical observations, provide administrative support and explain procedures to patients.

    Health and Wellbeing Coaches

    Health and Wellbeing Coaches use their skills to help people make conscious and informed health choices, change behaviours and encourage proactive management and prevention of illness.

    Clinical Pharmacists

    Clinical Pharmacists conduct structured medication reviews (SMRs), provide independent prescribing/deprescribing 
and give medication advice to healthcare professionals 
and patients.

    Pharmacy Technicians apply their pharmaceutical knowledge in tasks such as medicines reconciliation and prescription management support.

    Social Prescribers

    Social Prescribers connect people to non-medical community-based activities, groups and services in order to meet their practical, social and emotional needs (e.g. specialist advice services, the arts, physical activity and nature).

    Nursing Associates deliver hands-on, person-centred care, such as conducting clinical observations and clinical health checks, and support registered nurses who focus on more complex clinical care.

    Paramedics

    Paramedics in Primary Care can provide a rapid response to deteriorating patients and patients with long-term conditions, minor injuries and minor illness, as well as offering telephone triage and home visits.

    Family Navigators

    Family Navigators support families in accessing essential services and resources. They provide guidance, connect families to appropriate support systems, and help address challenges across parenting, mental health, education (+ more).

    Care Navigators

    Care Navigators are based in GP practices and support local residents with proactive care plans. They can also action community referrals to physiotherapists, district nurses or social services.

    Community Health & Wellbeing Workers (CHWWs)

    CHWWs are a local team who already live in the community and provide personalised, non-clinical support to improve health and connect people with local services, via home visits.
